MALDI-TOF MS技术快速检测产KPC酶肺炎克雷伯菌的研究Rapid Detection of Klebsiella Pneumoniae Strains Producing Carbapenemase by MALDI-TOF MS

摘要(Abstract):
应用常规细菌分离方法分离肺炎克雷伯菌,全自动微生物分析仪鉴定肺炎克雷伯菌并进行药物敏感试验,改良Hodge法检测菌株产碳青酶烯酶的能力,基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)检测产碳青霉烯酶(KPC)肺炎克雷伯菌和不产KPC酶肺炎克雷伯菌.结果显示,38株产KPC酶肺炎克雷伯菌对亚胺培南、美罗培南、头孢他啶、头孢吡肟、氨曲南和哌拉西林/他唑巴坦的耐药率为100.0%;产KPC酶肺炎克雷伯菌与不产KPC酶肺炎克雷伯菌的质谱峰不同,7个质谱峰的分布频率有统计学差异(P<0.01);质荷比为8 309.74和2 209.78处的质谱峰联合诊断产KPC酶肺炎克雷伯菌的敏感度为89.5%,特异性为96.3%.结果提示,MAALDI-TOF MS可以快速、准确地鉴别产KPC酶肺炎克雷伯菌.
